Patient Experience Coordinator

This is not a typical healthcare role, and we are not looking for a typical healthcare applicant. We are a relationship-based integrative medicine and Direct Primary Care practice in Centennial, CO. Our patients are not moving through fifteen-minute appointments. Many are navigating complex, chronic, or long-overlooked health concerns, and they have chosen us because they want to be heard, known, and supported over time. The person we hire will be one of the first voices these patients hear and one of the most consistent presences in their care.

We are seeking a Patient Experience Coordinator who treats patient relationships the way the best customer success managers treat their accounts: proactively, attentively, and with a real sense of ownership. Supporting both Lineage Wellness and Trivida Functional Medicine, this role is about making every patient feel known, supported, and never lost in the shuffle. What You Will Do
This role is built around relationships, communication, and coordination. Communication is the heart of it, and that is where most of your day will live.

Patient Relationship Management (your primary focus)

  • Serve as the main point of contact for patients across phone, portal, and in-person interactions
  • Manage a high volume of patient messages with clarity, warmth, and timeliness, with a standard of 24 business-hour response
  • De-escalate concerns and guide difficult conversations toward solutions
  • Build relationships over time so patients feel known and supported, not processed
  • Notice when a patient needs a check-in, a clarifying call, or a small extra step that turns a good experience into a great one
  • Be the steady, familiar voice patients come to trust

Scheduling and Care Coordination

  • Manage provider schedules across both practices, including new patient onboarding, follow-ups, and procedures
  • Coordinate appointments, labs, imaging, specialty referrals, and outside records
  • Track outstanding orders, results, and next steps so nothing slips between visits
  • Anticipate clinic flow and adjust proactively when the day shifts (and it will)
  • Ensure each patient leaves a visit knowing exactly what comes next and when

Documentation, Systems, and Team Collaboration

  • Maintain accurate, timely documentation in our EHR (Cerbo)
  • Use Spruce for patient communication and keep all threads clean and current
  • Uphold HIPAA compliance and patient confidentiality without exception
  • Collaborate closely with providers and teammates across both practices, communicating proactively and closing every loop
  • Contribute to a calm, organized environment where patients and coworkers both feel at ease
  • Receive feedback as part of how we get better together, and act on it quickly

What We Need From You

  • Two or more years in a role where relationships were your job; customer success, client services, account management, concierge, hospitality, executive assistant supporting clients, or similar
  • A track record of being the person clients or customers trust, return to, and refer others to
  • Strong written and verbal communication skills
  • High emotional intelligence, especially under pressure
  • Excellent organization and follow-through across a high volume of touchpoints
  • Comfort learning new software quickly (EHRs, communication platforms, scheduling tools)
  • Genuine curiosity about functional and integrative medicine, even if it is new to you

Helpful but not required: previous healthcare experience, familiarity with membership-based or relationship-driven service models (DPC, concierge medicine, private banking, boutique fitness, high-end hospitality, etc.) We are intentionally open to candidates without a clinical background. If you have spent your career making people feel taken care of, we want to hear from you.

Schedule, Location, and Pay
In-person in Centennial, CO
Monday through Friday, 7:00 AM to 4:00 PM, with a 45-minute lunch
$23.00 to $28.00 per hour, based on experience

At Trivida Functional Medicine and Lineage Wellness, our days are structured around longer appointments, complex cases, and weekly collaborative case reviews, because that is what root-cause care actually requires. Who You Will Care For

Our patients are engaged, invested in their own health, and ready to do the work. Many come to us with complex, multi-system conditions, including autoimmune disorders, gastrointestinal disease, hormone imbalance, and chronic inflammation. Others come for a more thoughtful, root-cause approach to long-term care. All of them are looking for a clinician with the time and training to think with them, listen carefully, and walk alongside them through the work.

The Clinician We Are Looking For Is Someone Who:

  • Thinks like a detective and listens like a friend
  • Is genuinely energized by complex cases that require time and patience to unpack
  • Has training, certification, or hard-earned experience in functional medicine
  • Wants longer, thoughtful appointments and accepts the responsibility that comes with them
  • Reads the journals, asks “but why,” and brings ideas back to the team
  • Practices with clinical confidence and intellectual humility at the same time Sees coaches, MAs, and admin teammates as essential partners, not support staff
  • Is in this work for the long haul, not as a stepping stone

What You Will Do
This role spans both practices, with the majority of your time in Trivida programs and the remainder in Lineage Wellness primary care. Functional Medicine Care (Trivida, 60-70% of focus)

  • Lead patients through structured 4 to 6 month root-cause programs
  • Synthesize complex histories across gut, hormone, immune, metabolic, and toxicity domains
  • Order and interpret advanced lab panels (Vibrant, Genova, ZRT, and conventional labs)
  • Develop and iterate personalized, layered protocols
  • Adjust treatment in real time based on patient response and emerging data
  • Walk patients through both the intensity of healing and the patience required to do it well

Direct Primary Care
(Lineage Wellness – about 30%)

  • Provide functionally informed primary care for patients in maintenance or optimization
  • Manage prescriptions, screenings, and acute concerns within a longer-relationship model
  • Support continuity for patients transitioning out of Trivida programs
  • Build the kind of trust that turns annual visits into long-term partnerships

Collaborative Clinical Work

  • Participate in weekly case reviews and protocol discussions
  • Bring fresh research, training, and clinical thinking back to the team
  • Contribute to the evolution of our protocols, not just follow them
  • Mentor and collaborate with health coaches, MAs, and other clinicians

Documentation and Systems

  • Maintain accurate, thoughtful documentation in Cerbo Use Spruce for patient communication, Doxy for telehealth, and Fullscript for supplements and protocols
  • Leverage clinical decision tools (Open Evidence, Heidi AI, UpToDate) as part of high-quality care

What We Need From You

  • Active NP or PA licensure in Colorado
  • Two or more years of clinical experience
  • Training, certification, or substantial self-directed study in functional medicine (IFM, A4M, or comparable)
  • Strong clinical judgment and willingness to make decisions in ambiguity
  • Genuine collaborative spirit, with confidence that does not need to be the loudest voice in the room

Helpful but not required: experience with hormones, GI, or autoimmune cases, familiarity with advanced lab interpretation, prior DPC, concierge, or membership-model experience.

Schedule, Structure, and Compensation

Full-time, 40 hours per week, primarily in-person in Centennial, CO

Hybrid flexibility may develop over time

Days are structured for depth, with longer appointments and balanced flow

Salary range: $110,000 to $135,000, based on experience and credentials
Monthly and quarterly performance bonuses

Revenue share opportunity after two years
Ownership pathway available within three to five years for the right clinician

A Note on Part-Time: We are hiring for one full-time provider, and that is our preferred path. That said, if two excellent part-time clinicians are the right people, we are open to building the team that way.
